국문 초록

본 연구는 중국 장기요양보험(Long-term Care Insurance, LTCI) 시범사업에서 나타나는 도시–농촌 간 구조적 격차를 분석하고, 이러한 차이가 제도의 형평성과 지속가능성에 미치는 함의를 평가하는 데 목적이 있다. 이를 위해 1차 시범지역인 칭다오, 상하이, 쑤저우를 중심으로 할당, 급여, 재원, 전달체계의 네 가지 정책요소를 비교 분석하였다. 문헌 및 정책문서 분석을 바탕으로 각 지역의 제도 설계와 운영 특성을 길버트와 스펙트의 정책분석틀에 따라 검토하였다. 분석 결과, 도시 지역은 상대적으로 높은 행정역량과 잘 구축된 서비스 인프라를 바탕으로 제도 운영에서 유리한 구조를 갖고 있는 반면, 농촌 지역은 서비스 접근성 부족과 자원 제약으로 인해 제도의 실질적 혜택을 얻기 어려운 것으로 나타났다. 이러한 결과는 장기요양보험의 전국적 확대 과정에서 지역 간의 불균형을 완화할 수 있는 농촌 맞춤형 급여 설계, 전달체계 강화, 형평성 있는 재정 조달 구조 등 정책적 조정이 필요함을 시사한다.

영문 초록

This study aims to examine structural disparities between urban and rural areas in China’s Long-Term Care Insurance (LTCI) pilot programs and assess their implications for equity and sustainability. Focusing on Qingdao, Shanghai, and Suzhou as first-round pilot cities, it compares four key policy components—allocation, benefits, financing, and delivery systems—using Gilbert and Specht’s policy analysis framework. Based on a systematic review of literature and policy documents, the analysis finds that urban areas benefit from stronger administrative capacity, denser service infrastructure, and more stable financing arrangements, whereas rural areas face constrained service access, limited resources, and weaker delivery systems. To address these gaps, the study proposes policy adjustments, including rural-tailored benefit designs, strengthened community-based delivery systems, and more equitable financing mechanisms. The study contributes to comparative social policy research by offering a structured urban–rural analysis within a single national framework and by extending the application of Gilbert and Specht’s model to multiple pilot cases. Its findings provide a theoretical basis for debates on equity and sustainability in the expansion of long-term care systems in transitional societies beyond China.
