국문 초록

간호사의 환자안전 리더십 역량을 강화하는 것은 지속가능한 환자안전 문화를 구축하는 데 핵심적이다. 본 주제범위고찰은 간호사 중심의 환자안전 리더십 프로그램의 설계, 실행 및 효과 평가 현황을 탐색하는 것을 목적으로 하였다. JBI 스코핑 리뷰 방법론에 따라 Cumulative Index to Nursing and Allied Health Literature, MEDLINE, Web of Science Core Collection 데이터베이스를 대상으로 2011년 7월부터 현재까지 출판된 문헌을 검색하였다. 연구 설계나 언어에 대한 제한은 두지 않았다. 중복 문헌을 제거한 후 총 4,553편의 문헌을 검토하였으며, 최종적으로 11편의 연구가 선정되었다. 검토된 11개의 프로그램 중 4개(36.4%)는 환자안전 관련 이론적 틀 또는 모델을 프로그램 설계에 반영하였으며, 4개(36.4%)는 집단 리더십을 강조한 팀 기반 접근법을 적용하였다. 또한 5개(45.5%) 프로그램은 현장 중심의 질 향상 프로젝트를 포함하였다. 평가 방법 측면에서, 8개(72.7%) 프로그램은 단일군 유사실험설계를 활용하였고, 환자 수준의 결과를 직접 평가한 프로그램은 없었다(0%). 이러한 결과는 이론적 근거를 기반으로 한 프로그램 설계와 무작위 대조군 연구를 포함한 엄격한 평가 연구의 필요성을 시사한다. 효과적인 간호사 중심 환자안전 리더십 프로그램 실행을 위해서는 조직 차원과 실무 단위 수준의 지원이 필수적이며, 효과 평가는 다차원적 관점을 반영해야 한다.

영문 초록

Empowering nurses to actively engage in patient safety leadership is vital for building a sustainable patient safety culture. This scoping review aimed to explore the design, implementation, and outcome evaluation of nurse-centered patient safety leadership programs. Following the JBI methodology for scoping reviews, a literature search was conducted in Cumulative Index to Nursing and Allied Health Literature, MEDLINE, and the Web of Science Core Collection for studies published from July 2011 to the present. No restrictions were placed on study design or language. After duplicates were removed, 4,553 records were screened, yielding 11 studies for the final review. Of the 11 programs reviewed, 4 (36.4%) explicitly incorporated patient safety–related theoretical frameworks or models into their design. Four programs (36.4%) adopted team-based approaches emphasizing collective leadership, while five (45.5%) incorporated on-site quality improvement projects. In terms of evaluation, Eight programs (72.7%) employed one-group quasi-experimental designs. No program directly assessed patient-level outcomes (0%). These findings highlight the need to apply theoretical frameworks and conduct rigorous evaluations, including randomized controlled trials (RCTs), to develop evidence-based, standardized leadership programs. Support at both organizational and unit-levels is essential for effective program implementation, and outcome evaluations should adopt multidimensional perspectives.
