Purpose: The purpose of this study is to examine Japan's healthcare and ‘health and productivity management’ policies and to explore their implications for Korea.
Research design, data, and methodology: This study at first reviews existing studies in Korea related to Japan's medical and healthcare policies and presents the differentiation points and analytical framework of this study. After that, it examines Japan's government-wide healthcare policies as well as those implemented by METI, and identifies the overall scale of Japan's healthcare industry. As to the Japan's Health and Productivity Management (HPM, Kenko Keiei) policy, this study explores the background of the policy and policy instruments along with case studies and management outcomes.
Results: This study first analyzed Japan's government-wide healthcare strategy. In Japan, the Headquarters for Healthcare and Medical Strategy Promotion under the Cabinet Office plays a central role in promoting health and medical policies at the national strategic level, in close coordination with MEXT, MHLW, METI etc. In particular, METI positions the healthcare sector as a pre-medical stage within the broader health and medical fields and seeks to foster it from an industrial perspective, promoting HPM as a core element of healthcare policy. Furthermore, METI has set concrete goals to expand the scale of Japan's healthcare industry, while simultaneously enhancing the international competitiveness of Japanese companies in the pharmaceutical and medical device markets. This study then focused on METI's healthcare policy, particularly its HPM initiative. The introduction of HPM in Japan stems from the government's intent to address the challenges of an aging population by encouraging companies to invest in employee health, thereby extending the nation's healthy life expectancy and fostering a “lifelong active workforce.” In doing so, the government aims to reduce social security costs, such as medical and long-term care expenses. Against this backdrop, the Japanese government introduced the Health and Productivity Stock Selection Program in 2015 and the Certified Health and Productivity Management Organization Program in 2017 as concrete policy instruments. Today, a large number of companies participate in these programs, reflecting their widespread success. The case studies of six companies demonstrate that Japanese firms are actively implementing HPM in line with government policy. Moreover, it was found that HPM exerts positive effects in multiple domains: securing investment in capital markets, attracting talent in labor markets, building favorable supplier relationships, obtaining national subsidies, and receiving various local incentives.
Implications: Based on the findings of this study, several implications can be drawn for Korea. From the government policy perspective, the study suggested the need to strengthen public awareness of rising social security costs and the importance of healthcare, to allocate greater financial resources to the pre-treatment stages of health maintenance and promotion, and to encourage active corporate participation in the Health-Friendly Certification system. From the corporate strategy perspective, it emphasized the necessity of promoting HPM from an internal standpoint such as securing outstanding human resources and from an external standpoint such as enhancing corporate image and also highlighted the importance of Korea-Japan collaboration in the healthcare sector.
