후두엽 신경낭미충증에 의한 동측반맹

국문 초록

목적: 좌측 후두엽 신경낭미충증에 의한 우측 동측반맹 소견을 보였기에 이를 보고하고자 한다. 증례요약: 20년 전부터 돼지 생고기를 섭취해 온 52세 남자 환자가 2주 전부터 발생하여 점점 심해지는 두통과 시야 흐림, 우측 반신의 감각 이상을 주소로 내원하였다. 나안시력은 양안 각각 1.0이었고, 험프리자동시야검사에서 우측 동측반맹을 보였다. 이외 안과 소견은 정상이었다. 뇌자기공명영상검사에서 좌측 후두엽에 고리조영증강이 되는 거대한 낭종과 뇌실질내 다른 작은 낭종들이 발견되었다. 혈청학적 검사에서 유구낭미충에 대한 항체 양성 반응을 보였다. 알벤다졸과 스테로이드를 2주간 투여하였으나, 시야의 호전을 보이지 않아 개두술과 낭종절제술을 시행하였고 이후 시야의 호전을 보였다. 결론: 돼지 생고기를 섭취한 과거력이 있는 환자가 동측반맹의 시야 이상과 신경학적 이상 증후를 보이는 경우, 드물지만 신경낭미충증의 가능성을 고려하여야 한다.

영문 초록

Purpose: To report a case of occipital neurocysticercosis presenting as homonymous hemianopia. Case summary: A 52-year-old male with a history of frequent raw pork ingestion for 20 years presented with headaches, blurred vision, and a tingling sensation in the right side that worsened during the past two weeks prior to his visit. Visual acuity was 20/20 in both eyes. Humphrey automatic perimetry showed right homonymous hemianopia. Other ophthalmic examination results were unremarkable. Brain magnetic resonance imaging showed a large cystic mass with ring enhancement in the left occipital lobe, and several small parenchymal cysts in the right hemisphere. A serologic test for neurocysticercosis with cerebrospinal fluid was positive. He was treated with albendazole and steroid for two weeks, but the visual symptoms persisted. Craniotomy and removal of the cystic lesion were performed, and the visual field improved. Conclusions: Symptomatic neurocysticercosis is rare, but it can be presented as homonymous hemianopia. Occipital lobe neurocysticercosis should therefore be considered when examining any patient with a visual field defect and a history of raw pork ingestion.
